Evaluation Measures

Primary

  • Evaluate the impact of Ayva on patients’ activation level (Patients must complete both Patient Activation Measure assessments and 80 percent of plan activity to be included).

Secondary

  • Evaluate the impact of Ayva on patients’ engagement
  • Evaluate the impact of Ayva on patient-reported ER visits and readmissions
  • Evaluate the impact of Ayva on patient-reported unplanned procedure cancellations

Requirements

  • Participants will join a kick-off call to review care plans and establish thresholds.
  • Participants agree to enroll at least 50% of eligible patients in the program.
  • Participants agree to complete a survey evaluating the program at the end.
  • Participants receive 80 hours of care plan customization. Additional customization is billed at $200 per hour.
  • Patients will be imported to Ayva via EHR integration or a manual CSV file import.
  • All work will be conducted remotely.
