Evaluation Measures
Primary
- Evaluate the impact of Ayva on patients’ activation level (Patients must complete both Patient Activation Measure assessments and 80 percent of plan activity to be included).
Secondary
- Evaluate the impact of Ayva on patients’ engagement
- Evaluate the impact of Ayva on patient-reported ER visits and readmissions
- Evaluate the impact of Ayva on patient-reported unplanned procedure cancellations
Requirements
- Participants will join a kick-off call to review care plans and establish thresholds.
- Participants agree to enroll at least 50% of eligible patients in the program.
- Participants agree to complete a survey evaluating the program at the end.
- Participants receive 80 hours of care plan customization. Additional customization is billed at $200 per hour.
- Patients will be imported to Ayva via EHR integration or a manual CSV file import.
- All work will be conducted remotely.
